We Take The Time To Get To Know You, Your Family And Support Network

The first step in our process is a detailed intake. We want to make sure we fully understand all of your wants and needs. We want to make sure we understand any specific cultural care preferences. During the intake process, we talk to you and your family about your goals and how we can best help you maintain your independence while also making sure you have immediate access to the care you need.

Once we understand what services you need, we present you with a service agreement. After the service agreement is completed, we match you with the best team members for your situation and get to work.

We value open communication and regularly check in with you to make sure you are getting the care you need and answer any questions or concerns that you may have.
